At CareSmart, we help individuals and families navigate Georgia’s Medicaid waiver programs so they can receive quality care in the community, rather than in a nursing facility.
We specialize in case management for the Community Care Services Program (CCSP) and the Service Options Using Resources in a Community Environment (SOURCE) program. These Medicaid-funded waiver programs support individuals with chronic illnesses, disabilities, or age-related needs who qualify for nursing home-level care but prefer to remain at home.

Who Qualifies

How to Get Started
Referrals to these programs are usually submitted through the Area Agency on Aging. Once referred and approved, CareSmart’s case management team works closely with members and their families to develop a care plan, coordinate services, and monitor ongoing needs.
