At CareSmart, we provide Traditional and Enhanced Case Management to Medicaid members enrolled in the Community Care Services Program (CCSP) and Elderly and Disabled Waiver Program (EDWP) across the following Georgia counties:
Our goal is to ensure each member receives safe, effective, and personalized care at home or in their community. Our services include: Comprehensive Case Management Develop and maintain individualized Comprehensive Care Plans (CCPs) Monitor members’ health and adjust care plans as needed Coordinate services across multiple providers to ensure continuity of care Ensure services are delivered as ordered and meet quality standards
Service Coordination & Resource Linkage Broker Medicaid waiver services (e.g., personal care, skilled nursing, adult day health) with approved providers Assist with access to additional resources, including: Medical transportation Home-delivered meals Durable medical equipment (DME) Non-Medicaid community resources
Client Advocacy & Support Act as a liaison and advocate for clients within the healthcare and human services system Support members through the appeals process related to service denials or reductions Engage with members and caregivers regularly to understand preferences and evolving needs
Crisis Response & Quality Oversight Identify and resolve issues related to service delays, quality of care, or unmet needs Report and act on incidents of abuse, neglect, or exploitation Provide discharge planning in the event of death, relocation, or significant life changes
