For over a decade Caresmart has provided case management services to the elderly and disabled in Georgia. This experience has provided Caresmart Care associates with the opportunity to hone knowledge, skills and abilities as they relate to the coordination of services throughout the spectrum of healthcare.

CareSmart Case Management agency provides services to Medicaid Waiver program beneficiaries, through a team of case managers who assist the elderly or individuals with needs for care outside a care facility such as a Nursing Home or Over the years, the Georgia Department of Community Health (DCH) that Administers the Medicaid Program in Georgia on behalf of the Federal Government has been facing challenges that have led to a long list of potential beneficiaries being placed on the waiting list for months on end. These challenges became compounded with the onset of the pandemic in early 2020.

The state program facilitators such as Department of Community Health, Department of Human Services and Department of Children and family services in addition to several case management companies have made efforts to resolve some of these challenges yet issues remain. These barriers include lack of funds, unclear scope of practice, diverse and complex case management activities, insufficient staff training, poor collaboration with healthcare providers and other social services organizations in the industry, client relationship challenges and poor remuneration for case managers.

CareSmart Case management was established in late 2021 in Atlanta Georgia to help overcome or reduce the impact of some of these challenges through practical approach to case management using standardized evidence-based protocols and certification programs, working with health policy makers, case management associations and health-care researchers which will culminate in improved case management practices across the board.

We at CareSmart Case Management help individuals to navigate the complex maze of Medicaid waiver programs in Georgia. These programs are meant to provide care for people with disabilities and/or chronic health challenges in the community. The waivers allow healthcare professionals to provide care in a waiver member’s home in the community instead of a long-term care facility. The Community Care Services Program (CCSP) and Service Options Using Resources in a Community Environment (SOURCE) program are Medicaid home and community-based waiver services programs that provide community-based social, health and support services to eligible consumers as an alternative to placement in a nursing home
facility.
