CareSmart is a Georgia-based case management agency committed to supporting elderly and disabled individuals through compassionate, coordinated care. While our company was officially established in 2021, our team brings years of combined experience in navigating the complexities of Medicaid waiver programs and community-based services.

Founded in Atlanta, CareSmart was created to help reduce the challenges many face when accessing long-term care at home. We respond to the evolving needs of our clients by applying evidence-based practices, continuous staff training, and a personalized approach to care.
